Position Overview
EmpowerBridge is seeking a compassionate and dedicated Social Worker / Case Manager to support individuals facing social, economic, and health-related challenges. The successful candidate will work closely with clients to assess their needs, develop personalized support plans, and connect them with appropriate community resources and services.
Key Responsibilities

Conduct client assessments and develop individualized support or case management plans

Provide guidance, counselling, and crisis intervention when needed

Connect clients with community resources such as housing, healthcare, employment, and social services

Advocate for clients’ rights and promote access to essential services

Maintain accurate case records and documentation

Collaborate with healthcare providers, social service agencies, and community partners

Support individuals experiencing barriers related to disability, addiction, or settlement challenges
Qualifications

Degree or diploma in Social Work, Human Services, or a related field
